Youth Consignment Form Registration

A parent/guardian of an underage consignor shall execute the consignment form provided by PCLA. A consigner who is of age to sign for themselves shall provide proof of age to the Board of Directors at the time of consignment. Any unpaid debt from the PCLA Sponsored Youth Livestock Sale up to 5% of the most recent annual gross sale shall be paid by PCLA until it is collected from the buyer(s). The Board of Directors may adjust this percentage if unforeseen circumstances outside of the control of the Board of Directors justify such action. The Board of Directors may establish exceptions to this rule if PCLA’s financial circumstances allow said action and two-thirds (2/3) of the directors vote in favor of adjustment.

1. The purpose of the Preston County Livestock Association shall be operated as a nonprofit organization and shall be operated exclusively for agricultural purposes within the meaning of Section 501 (c)(5) of the Internal Revenue Code, or the corresponding section of any future Federal tax code. PCLA seeks to work to educate local youth in agriculture and generally promote agriculture. PCLA seeks to work in conjunction with local 4-H and FFA groups to ensure that the junior membership of PCLA has an annual youth show and sale for their consigned market livestock projects. We believe in creating a safe environment that encourages educating youth in agriculture. All PCLA members participating in or
attending events, meetings, activities, livestock shows, or sales sponsored by PCLA, are required to conduct themselves according to the current PCLA Bylaws, Rules and PCLA Member Code of Conduct and future modifications or amendments to the PCLA’s Member Code of Conduct.
2. I am aware that my actions and decisions affect me and others. I acknowledge that I have been given a copy of the PCLA Member Code of
Conduct. I agree to conduct myself in accordance with the PCLA Member Code of Conduct at all times. I understand that my failure to do so may result in the loss of privileges during PCLA events, meetings, activities, livestock shows or sales including involvement in future PCLA events, meetings, activities, livestock shows or sales. My signature below demonstrates my agreement to abide by the PCLA Members Code of Conduct, as well as any code of conduct established for a specific PCLA event I attend.

The novel coronavirus, Covid-19, has been declared a worldwide pandemic by the World Health Organization. Covid-19 is extremely contagious and is believed to spread from person-to-
person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing. The Preston County Livestock Association (PCLA), PCLA officers, board of directors, members, volunteers, and other attendees cannot guarantee that you (exhibitor/parent/guardian/spectator/buyer) will not become infected with Covid-19. Further, attending any event hosted by the Preston County Livestock Association during the 2024-2025 year could increase your risk of exposure of contracting Covid-19. By signing this agreement, I acknowledge the contagious nature of Covid-19 and voluntarily assume the risk that I (exhibitor/parent/guardian/spectator) may be exposed to or infected by Covid-
19 by attending any event and/or meeting of the Preston County Livestock Association and that such exposure or infection may result from the actions, omissions, or negligence of myself and others, including but not limited to, committee members, volunteers, and program participants and their families. I voluntarily agree to assume all the foregoing risks and accept sole responsibility for any injury to myself
(exhibitor/parent/guardian/spectator) including but not limited to personal injury, illness,
disability, death, loss, claim, liability, or expense, of any kind, that I (exhibitor/parent/guardian/spectator) may experience or incur in connection with my attendance and/or participation at the Preston County Livestock Association event. On behalf
of myself (exhibitor/parent/guardian/spectator), I hereby release, covenant not to sue, discharge, and hold harmless the Preston County Livestock Association, PCLA officers, board of directors, members, volunteers, and other attendees, of and from the Claims, including all liabilities, claims, actions, damages, costs, or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Preston County Livestock Association, PCLA officers, board of directors, members, volunteers, and other attendees,
whether a Covid-19 infection occurs before, during, or after participation in the Preston County Livestock Association event.
